Definition
Physical location and time of actual occurrence of problems in the problem resolution process survey.
In detail
Point of Occurrence (POO) is the physical location and time where a problem actually occurred, and is a link in the causal chain continuum. Unlike the Point of Recognition (where the problem was noticed) and the Point of Cause (where the root cause lies), POO is the direct site of problem generation. It helps teams distinguish between problem symptoms and root causes, avoiding wasted effort in the wrong place. Typically, it outputs a specific location and time to guide on-site investigation.
Example in practice
A certain equipment failure caused downtime. The detection point was an alarm in the control room, while the occurrence point was the motor on the shop floor. An engineer went to the occurrence point, found bearing wear, replaced it, and restored operation. By recording POO, data was provided for future preventive maintenance.
